[Fontinst] T1/fcd/m/sc has wrong glyph for \v{z}, ! in \~{[C-LR-Zc-LR-Z]}

I seem to have some proof: Junicode. This font comes in Regular, 
Italic, Bold, and Bold-Italic. Regular has real SmallCaps, Bold not. 
Again fontsmpl.tex shows for the real SmallCaps, i.e. fjurc8t, the 
mistakes, in bold with fjubc8t no mistake. They are repeated in 
testfont.tex.
